Step 1: Register

If you have Adobe Acrobat Professional, follow these steps:
  • Complete the form & print a copy for your records
  • Save the completed form
  • Send the form as an email attachment to powelst@auburn.edu

If you do not have Adobe Acrobat Professional, you will not be able to save the completed form. Instead, follow these steps:
  • Complete the form & print two copies
  • Keep one copy for your records
